Proposal processing forms:

Per University policies, at least two forms are required for proposal submission:

The Conflict of Interest Screening form should be completed and signed by each Kent State University employee named in a proposal, then returned to the Sponsored Programs office in 134 Cartwright Hall.

The
Transmittal Form for Proposal Review and Approval should be completed and signed prior to proposal submission. If the proposal involves research, the research discipline designation section of the form needs to be completed as well. If the proposal is primarily instruction or public service-related, this section can be left blank. 
The transmittal form lists the PI and all co-PIs who are Kent State University employees, and should be signed by all of them, along with their respective Chairs and Deans. For regional campus proposals the campus Dean signs, as does the Executive Dean for Regional Campuses. Once the form is completed and signed by all parties, it should be returned to the Sponsored Programs office in 134 Cartwright Hall.

NIH applications also require a PHS Assurance form and a Pubmed Central form which will be kept on file in the Sponsored Programs office with the application. These forms should be completed and signed by the PI.
