When to Use This Form |

Request to Use Animals |
You must complete this form to be sure that the regulatory requirements are met when live vertebrate animals are to be used in a project at Kent State University. IACUC must review all research, teaching, and testing activities involving the vertebrate animals. |

Participant Qualifications |
The "Request for Use of Animals in Research" form lets you enter the qualifications for four project participants. If you are seeking approval for a protocol that will have more than four persons working on the project, you must complete this form, attach it to the protocol request, and submit it to the IACUC coordinator before IACUC approval of the protocol can be granted. All study personnel must provide documentation of IACUC training and enrollment in the occupational health program. |

Addition of Personnel |
Complete this form for each person that you want to add to an approved protocol that involves the use of animals. All new study personnel must provide documentation of IACUC training and enrollment in the occupational health program. |

Request for Animal Use Involving Hazardous Materials |
Complete this form when you want to conduct research that involves animals and biohazardous agents. |

Modification of Existing Protocol |
You must complete this form if you want to propose procedural changes or additions that involve living animals to an approved "Request to Use Animals" protocol. If you want to add personnel to an approved "Request to Use Animals" protocol, complete the "Addition of Personnel" form. |

Annual Review |
You must complete this form annually if you are seeking renewal of a "Request to Use Animals" protocol. If your project continues longer than three years, you must complete a new "Request to Use Animals" form and submit it to IACUC before the three-year anniversary of the original approval. Requests must be submitted to IACUC at least 30 days prior to protocol expiration date. For questions about your protocol expiration date contact Paulette Washko (330.672.2074) |

You must review the information contained in the Occupational Health and Safety Program and then complete this form to be able to work on an approved Institutional Animal Care and Use protocol at Kent State University. You must return the completed form to: KSU Research Compliance Office

| This document should be taken to the University Health Center for completion, if you have chosen full participation in the Occupational Health Program. The completed form should then be returned to the Research Compliance Office, Cartwright Hall Office #137. This document provides you with a checklist to verify that you have obtained and completed all of the steps for the Occupational Health Program for individuals involved in the care and use of animals at Kent State University. | |
| Animal Care and Use Questionnaire | Your occupational health needs in relation to your role in the animal care and use program will be individually assessed if you have chosen full participation in the Occupational Health Program. Take a copy of this completed form with you to your visit with the University Health Center to assist the health care professional in identifying your occupational health needs. After your visit to a health care professional, you must submit this completed form to the Office of Research Compliance Cartwright Hall, Office # 137. |
